Nurse Case Coordinators assume the primary responsibility for coordinating health care needs for students with medically complex care needs in school divisions and private schools within the Central Alberta RCSD boundaries. This involves organizing health, school, and community resources that are needed to assist students to function as independently as possible in order to maximize their school day.
The role of the Nurse Case Coordinator is multi-faceted. Based on the student, family, and school’s situation, there may be short-term or long-term involvement, and ongoing monitoring of the student’s medical condition to determine services required. The role of the Nurse Case Coordinator is multi-faceted. Based on the student, family, and school’s situation, there may be short-term or long-term involvement, and ongoing monitoring of the student’s medical condition to determine services required. The following services may be accessed to assist the medically complex student:
Services
- receives referrals from the schools, Children’s Rehabilitation Services (CRS) therapists, Home Care, Public Health, Diabetes Education Program, Family Support for Children with Disabilities (F.C.S.D), and other community agencies that have concerns about a student with a medical condition;
- navigates the health care system, and collaborates with health care professionals and service providers to coordinate care for a medically complex student;
- establishes a collaborative care plan based on the needs of the student with the involvement of the student, family, school and other service providers, with an ongoing evaluation of the client care goals;
- advocates on behalf of students, their families, and/or schools to ensure necessary resources/supports are available from the health care system, community resources and government agencies.
- supports, teaches, and provides in-services and/or resources to the student, parents, care providers or school personnel on a variety of topics (such as diabetes, tube feeding, the use of epi-pens, etc.) to ensure that safe and competent care is provided for the student at school.
- identifies and offers best practice protocols that avoid potential risks for procedures for the child that are completed by school staff (G-tube feeds, catheterizations, etc).
