Transition of PUF Students
to Grade One
There are many PUF children annually who transition to grade one in September. Not all children who were receiving Program Unit Funding will require individualized services if supports and strategies are already in place. Some, however, will continue to require individualized services to support their programming in the new classroom. In order to ensure that the services are available for the students who need them in early September, please consider your selection carefully.
Schools are encouraged to discuss the transition from PUF to Grade 1 with the parents early in the spring of kindergarten and are invited to use the sample parent letter (attached) to clearly communicate the possibilities for service in grade one.
Steps in the transition process:
Schools are encouraged to discuss the transition from PUF to Grade 1 with the parents early in the spring of kindergarten and are invited to use the sample parent letter (attached) to clearly communicate the possibilities for service in grade one.
Steps in the transition process:
- School personnel and the PUF service provision team will discuss if a child will likely require individualized services in grade one. In many cases the existing strategies and recommendations from the PUF team will continue to be valid in grade one, therefore, a referral for individualized services is not required. (In this case, it is possible to refer in the fall should a concern be identified by the school team at that point.)
- If it is anticipated that individualized services will be required, please invite the core team service providers to the transition case conference. If the child is changing schools, it may be necessary to contact the receiving school for the names and contact information of the SLP and OT who are assigned to the receiving school so that they can be invited to the case conference.
- Following the case conference, if it is determined that the child will need individualized services, please completed the CA RCSD referral http://carcsd.ca/resources/forms-and-templates/ and submit it to AHS Coordinated Intake by June 30 with Year End Assessment and Reports.
