How to make Educational Psychological Assessment Referral
Part A:
Teacher (with support from the SSF) makes a referral to the Program Planning Team (PPT) which addresses the following about this student:

- (2) What assessments have been completed

- (1) PPT Meeting should include Administration, SSF and teacher, but may also include other relevant stakeholders for the student (e.g., EA, parent/guardian, Family Wellness, etc.).
- (2) During the PPT meeting, the team may recommend new/unique in-school interventions/modifications that have not been tried yet.
Part C:
When a decision is made to refer the student for formalized psycho-educational testing, contact Kathy Murch (Director of Student Services) to determine who will be completing the assessment. Option could include:
A) Randall Symes
B) Private Psychologist contracted by the division
C) Multidisciplinary Centre Such As Glenrose, ASPIRE, Camrose Clinic

Part E:
- Assessing Psychologist or Assessment centre will contact SSF.
- SSF Confirms Date with Family: The SSF will then contact the student's parent(s)/guardian(s) with the date to ensure that they can meet with the psychologist post assessment. Once they have confirmed attendance, then SSF will let Randall Symes know that their proposed date works for the family.
- Additional Assessment Forms (BASC/ABAS): Sometimes additional forms will be required to be completed by the family; and (2) the teacher. SSF will pass these along to the teacher and the parents, and upon their return, will direct to assessment centre or psychologist.
- SSF Confirms Date with Teacher(s): Next, the SSF will inform the teacher(s) of the confirmed assessment date. Let them know that the student will not be in class during the time of the assessment.
- What to do on the Date of Assessment: When the psychologist arrives, SSF will provide him/her with any additional paperwork they wish to review. For example, they often want to take a look at the students CUM file for historical assessments, reports, report cards. If applicable, they may look at the red binder as well.
- Provide Space & Gather Student: SSF will provide a quiet space that the psychologist can do their assessment, and SSF will get student from their class.
- Post Assessment Meeting with Family & Psychologist: If it is possible, the SSF should be present at the meeting. In addition, depending on the situation, an administrator's presence may be warranted (e.g., in cases when familial situations may be volatile).
